What is West Moreton Care Connect?

When a loved one develops a serious illness, it’s normal to go through an emotional experience similar to grieving.

If the illness is terminal, the fears and feelings that surface can often overcome individuals and leave them feeling lost and helpless.

This is why the team at West Moreton Region Care at the End of Life Collaborative developed the website WestMoretonCareConnect.com.au.

After feedback from the community detailed how the end of life care could be improved,  the website acts as a response designed “for people with a life-limiting illness, their families and carers, to help plan and prepare for end of life.” 

The West Moreton Care Connect website gives community members “access to local services that are unique to the West Moreton region, on top of national support service information”.

Essentially, it is a “directory of local services, support networks and organisations that can assist people reaching the end of their life, and their carers”.

The End of Life Collaborative was set up in 2018 to address gaps in care and access as well as the capability of health workers and professional caregivers.

“The collaborative is made up of Blue Care, CiMaS Home Nursing Service, Darling Downs, and West Moreton PHN, Icon Cancer Centre, Ipswich Hospice Care, Mater Private Hospital Springfield, Palliative Care Queensland, Queensland Ambulance Service, St Andrew’s Ipswich Private Hospital and West Moreton Health.”

Ipswich Hospice Director of Nursing Carol Hope explains how the “website relieves families of all that onerous legwork and presents information about all the great resources and services that exist in West Moreton in one simple location.”

As the information is in one simple location, it reduces stresses surrounding the practical aspect of end-of-life planning. 

It has information from the community and residential aged care facilities to services that will help with gardening or provide meals.

All in all, it is a great platform connecting people to support services when they most need it!
