Declaration by Applicant
I have read and understood the funding conditions attached to this grant and agree to abide by those conditions. Specifically:
- Upon completion of the project, a report including evaluation and a photo should be submitted to the foundation (not more than 100 words)
- An expenditure statement with copies of invoices and receipts for the amount of the grant. For examples: invoice for fees, receipts for travel, accommodation etc.
- Attendance and/ or presentation at Foundation functions if requested.
- Immediate notification to the Foundation of major changes to the proposed conference. The Foundation reserves the right to modify or suspend funding at its own discretion if changes to the proposed activity do occur.
- Assume all responsibility for any tax payments that result from receiving a grant.
- Notification of change of contact details.
- I also give permission for the Ipswich Hospital Foundation to include information on the award of this grant in its publications and media releases.
You also need to provide hard copies of the following:
- Business Plan outlining expenditures, revenue, etc
- Written endorsement (or e-mail) by relevant Executive Director for WMDHS projects or organisation Director if from another organisation.
- Letter (or e-mail) from Business Unit Manager indicating that proposed project is outside of normal budgetary means.
- Brochures or other information describing the proposed piece of equipment or activity.
